Our dedicated multidisciplinary allied health team deliver goal-oriented therapy designed around you & your wellbeing. To get back into the community, you need to be treated in the community.

Our service allows you to:

  • Enhance your community engagement
  • Transition back into your everyday life
  • Build your independence whilst remaining active in the community
  • Work towards your individual goals with support every step of the way
  • Build up your confidence in social & environmental situations

We do not just treat an illness; we treat the person. We want to make sure you thrive within the community. Participating in community rehab gives you the space to improve in your preferred environments, such as out shopping, in parks, & around the home.

That is the community-based difference.

Eric’s story

In the days leading up to his surgery, Eric and his wife went on a trip to Ingham. He describes the time before like a ‘clank’ -he did not remember going. Recovery has been a long journey for Eric, but he has since returned to Ingham and his wife helped him to retrace their steps.

At the beginning of his rehabilitation, his main goal was to return to driving and play lawn bowls. Now, what he enjoys most is the little things, like eating mango ice-cream and going grocery shopping. He remembers sitting outside the shops while his wife bought groceries, unable to help, and now, he can do it himself again.

“My memory and speech were gone and I found it difficult to communicate with people or remember what to say,” he explained, but he’s learnt to take his time to think about what he wants to say and not rush into it. “I learned to realise everyone takes the time to think about what they want to say.”

Eric has been spending his time after rehab catching up on the things he missed out on, which is keeping him active, like working on his caravan. “Now that I can drive again, we are taking a trip to Maroochydore.”

Eric is much happier and more confident, but he said he owes it all to his wife. “I couldn’t have done it without her – I really didn’t want to get help but I’m grateful she pushed me to try rehab.”

Doreen’s Story

Doreen was referred for Community Based Rehabilitation because her nursing home could not offer a frequent, intensive service. She found she was using her wheelie walker for everything in her home and wanted to be able to walk without it aiding her.

“I want to be able to walk out the door and to my dining room without my walker”.

Since participating in frequent physiotherapy, she can now move longer distances without her wheelie walker and is more confident at home.

She said she has surprised herself with how well she can accomplish things now,” I can do a lot better than what I could do before.”

Although she still uses her wheelie walker, she can move longer distances without it and is confident to continue her home exercise plan independently, as well as with the assistance of nursing home staff.

Her new goal? “I just want to be able to put my knickers on without help – and after physio I can reach and stretch a lot better.”
