Our multi-disciplinary team will work to help participants understand, manage and improve their health to manage their condition and prevent associated complications.

With healthy lifestyle choices evidence shows type 2 diabetes can be prevented or delayed in up to 58% of cases.

Credentialed Diabetes Educators

A Credentialed Diabetes Educator (CDE) plays an important role in your journey. From first diagnosis, a CDE explain what diabetes is and provide individualised blood glucose advice and how to maintain your appropriate levels. They assist with tests and screenings for complications which are suitable for you condition, your age and your lifestyle.

A CDE can also help with the prescription or advice on a range of equipment:

  • blood glucose testing strips / consumables
  • continuous glucose monitoring
  • lancing devices
  • hypo treatment
  • travel bags for equipment transit

Accredited Practicing Dietitians

Accredited Practicing Dietitians to provide healthy eating plans for weight control. Our dietitians can tailor realistic plans which will encourage them to enjoy their diet while maintaining good health.

Exercise physiologists

Exercise Physiologists can plan and deliver  tailored programs for aerobic, strength and balance based exercise plans.

Counsellors

We have Psychology and Social Work service available to provide emotional support and link in with community support groups.
